Cancellation Policy

Unfortunately, Wix does not allow people to self cancel. To cancel, simply send a text to this number: 203-303-4725, indicating your name and the time and date of your appt. You can also forward your confirmation or reminder email to this email address: lifestyleandfamilymedicine@gmail.com with a note that says you'd like to cancel that appointment. To reschedule, simply go to the LFM website (linked below) and book another time.
